基本医疗保险是为补偿劳动者因疾病风险造成的经济损失而建立的一项社会保险制度。通过用人单位和个人缴费，建立医疗保险基金，参保人员患病就诊发生医疗费用后，由医疗保险经办机构给予一定的经济补偿，以避免或减轻劳动者因患病、治疗等所带来的经济风险。基本医疗保险是社会保险制度中最重要的险种之一，它与基本养老保险、工伤保险、失业保险、生育保险等共同构成现代社会保险制度。从1949年至今，我国基本医疗保险制度的改革与发展大致经历了三个阶段：第一阶段，从建国之初到改革开放前期，主要是公费、劳保制度的建立和运行；第二阶段，从改革开放初期到1994年期间，在当时经济改革的形势下，进行了有计划、分步骤的医疗保障制度全面改革的局部试点的经验；第三阶段，1998年，我国城镇职工基本医疗保险制度建立（寸洪斌，2014：37）。1998年12月，国务院发布《关于建立城镇职工基本医疗保险制度的决定》，要求在全国范围内建立以城镇职工基本医疗保险制度为核心的多层次的医疗保障体系。该《决定》指出，医疗保险制度改革的主要任务是建立城镇职工基本医疗保险制度，即适应社会主义市场经济体制，根据财政、企业和个人承受能力，建立保障职工基本医疗需求的社会医疗保险制度。
